Wrong disposal of in-patient medical records
********************************************

The following is issued on behalf of the Hospital Authority:

The spokesperson for Our Lady of Maryknoll Hospital made the following announcement today (November 8):

The hospital today reported to Hospital Authority (HA) Head Office an incident of wrong disposal of printed copies of 4 271 in-patient records. As all key information of in-patient admissions has been saved in the HA Clinical Management System, the subsequent clinical management of patients will not be affected.

During the processing of a patient request on medical record in August this year, a staff member reported to the management that some printed copies of in-patient records have been wrongly disposed of. The hospital sought assistance immediately from the Head Office Information Technology Division in ascertaining the number of wrongly disposed patient records. As proper procedures in disposal of confidential document have been followed in the destruction of the patient records, there has been no leakage of patient information.

The hospital expressed concern and apology towards the incident and would contact the  patients concerned by mail apart from setting up an enquiry hotline 2354 2426.

Kowloon West Cluster would set up panel to conduct an investigation of this incident and a report would be submitted to the HA Head Office in eight weeks.
