AAIA publishes investigation report on serious incident involving Boeing 747-8 freighter

    ​The Air Accident Investigation Authority (AAIA) today (December 7) published the investigation report (8/2022) on the investigation into a serious incident involving a Boeing 747-8 freighter (registration mark N624UP) at Hong Kong International Airport (HKIA) on July 20, 2021.
      
     At 12.16pm on the day of the incident, the freighter took off from HKIA to Dubai International Airport. About four minutes later, the flight crew shut down the No. 1 engine due to excessive engine speed, and elected to return to Hong Kong. After landing on the then-North Runway, the No. 1 engine caught fire. There was thermal damage to the engine of the freighter but no one was injured in the incident.
      
     The investigation was conducted by a team of professional investigators in strict adherence to international standards established by the International Civil Aviation Organization (ICAO). The investigation concluded that improper installation of a fuel supply pressure (P1) bypass valve port fitting on the fuel metering unit resulted in a fuel leak that rendered the engine fire. The investigation team has made two safety recommendations and the relevant parties have taken follow-up actions.
      
     "The objective of the investigation was to identify the circumstances and causes of the serious incident with a view to preventing a recurrence," an AAIA spokesperson said.

     The AAIA is an independent investigation authority responsible for the investigation of civil aircraft accidents and serious incidents in accordance with the Hong Kong Civil Aviation (Investigation of Accidents) Regulations (Cap. 448B) and with reference to ICAO's standards.
