Queen Mary Hospital announces sentinel event

The following is issued on behalf of the Hospital Authority:

     The spokesman for Queen Mary Hospital (QMH) announces a sentinel event today (August 20):

     A 73-year-old female terminal cancer patient was admitted to an oncology ward in QMH earlier for palliative care. Patient agreed for Do-Not-Attempt Cardiopulmonary Resuscitation previously. Appropriate treatment was provided during her stay in the hospital.

     A ward staff last observed the patient at 6.30am this morning. The patient was awake and emotionally stable. Around 15 minutes later, the patient was found unconscious with a plastic bag covering her head and some drugs were found aside. Ward staff immediately removed the plastic bag and provided supportive care. The family members were also informed. Her condition continued to deteriorate and was certified dead at 4.24pm.

     QMH is deeply saddened by the incident. The hospital expresses condolences to the family members and will offer necessary assistance. The incident has been reported to the Hospital Authority Head Office via the Advance Incident Reporting System. The case has been reported to the Police for investigation and referred to the Coroner for follow-up.
