A new user-friendly joint office staffed by the Immigration Department(ID), the Department of Health (DH), and the Food and Environmental Hygiene Department (FEHD) will open on Monday (October 30) to handle death registration and cremation-related matters.
The joint office at the first floor of Cheung Sha Wan Government Offices, 303 Cheung Sha Wan Road, Kowloon, will house three units, namely:
1. Births and Deaths General Register Office (Kowloon Deaths Registry) of ID,
2. Port Health Travel Health Centre (Kowloon) and Cremation Permit Unit of DH, and
3. Cremation Booking Office of FEHD.
The new office, which is conveniently located near exit C of Sham Shui Po MTR station, will operate from 9 am to 4.30 pm on weekdays and 9 am to 12.30 pm on Saturdays.
Members of the public visiting the joint office can apply for:
1. death registration, applications for search of death record or certified copies of death certificate from ID (Tel. 2368 4706);
2. travel health service (consultation, health risk assessment, health advice, provision of travel-related immunisations and medications) (Tel. 2150 7235)and issue of cremation permits (Tel. 2150 7232) from DH; and
3. bookings of cremation sessions at FEHD's six public crematoria at Cape Collinson, Diamond Hill, Fu Shan, Wo Hop Shek, Kwai Chung and Cheung Chau (Tel. 2150 7503).
Meanwhile, the respective units of these departments at the Canton Road Government Offices will cease operation after office hours today (October 28).
Persons who have already applied for search of death record and/or certified copies of death certificate in the old Canton Road office can continue to collect the result of search or certificates at the new office from Monday onwards.
With effect from Monday, the Immigration Department's Queen's Road East Office at Wu Chung House, Wan Chai will be renamed as the Births and Deaths General Register Office (Hong Kong Island Deaths Registry). The office name for the existing Canton Road Office will be changed to the Births and Deaths General Register Office (Kowloon Deaths Registry) on its removal to the new office in Cheung Sha Wan Government Offices.
