Kowloon Central Cluster announced medication incident

The following is issued on behalf of the Hospital Authority:

     The spokesperson for the Kowloon Central Cluster (KCC) made the following announcement today (June 2) regarding a medication incident reported by the Hung Hom Clinic (HHC):

     On May 26, a pharmacist of HHC found two tablets of 10 milligrams anti-hypertensive drug (Amlodipine) inside a pre-packaged pack of medication, which should contain 112 tablets of 500mg pain-relief drug (Paracetamol) before issuing to patient.

     The clinic immediately checked the dispensing record and the prepacking record and confirmed that the involved Paracetamol tablets were pre-packaged on April 27. HHC immediately inspected the remaining stocks of Paracetamol and Amlodipine and confirmed there was no mixing of medication. The clinic also promptly called a total of 63 patients who may be affected in the incident to check if they found other tablets mixed in the Paracetamol dispended to them, and the patients were also asked about their health conditions.

     The pharmacy of the clinic had reported the incident to the KCC management. Having assessed the incident, the Cluster Family Medicine and Primary Health Care department decided to arrange the concerned 63 patients to return to HHC for a health check-up and bring back the medicines for checking starting today as a prudent measure. 62 patients have been contacted and no mixing of medication nor any discomfort have been reported so far.

     KCC apologised to all the concerned patients for the incident and would continue to closely communicate with them and provide necessary assistance. The Cluster had also reported the incident to the Hospital Authority Head Office (HAHO) through the Advance Incident Reporting System. The Cluster would investigate the incident, review the prepacking process and the reporting of medication incident and make recommendations to prevent recurrence of similar incident. An investigation report would be submitted to HAHO. Immediately after the incident, the pharmacy of the clinic has already reminded all staff members to be vigilant and to strictly observe the established procedure on preparation of medications.
