The Mandatory Provident Fund Schemes Authority (MPFA) today (September 21) reminded scheme members that if they considered themselves eligible for the Government's injection of $6,000 but had not yet received the injection from the trustees concerned, they had to lodge requests for review of eligibility on or before next Wednesday (September 30).
An MPFA spokesperson said that for scheme members who wished to request for review of eligibility, they should call the Injection Hotline 2926 6000, write to the MPFA or visit the Government Injection Project Office at the MPFA's Kwun Tong office in person (Address: 25/F, Tower 1, Millennium City 1, 388 Kwun Tong Road, Kwun Tong, Kowloon).
The spokesperson emphasised that scheme members must lodge their requests for review of eligibility on or before September 30. Any late request for review of eligibility would not be considered.
When lodging review requests, scheme members are required to provide identification documentation and income proof, and other relevant information (e.g. photocopy of Hong Kong ID card, employment contract, MPF membership certificate, payroll slip, Annual Benefit Statement, proof of residential address).
