Regarding a recent medication incident in Tung Wah Hospital (TWH), the TWH spokesperson made the following announcement today (April 21):
TWH is very concerned about the incident and has set up an Independent Investigation Panel to investigate the circumstances surrounding the medication incident and make recommendations to prevent recurrence of similar incidents.
The Independent Investigation Panel will be chaired by Hospital Chief Executive of Grantham Hospital, Dr Joseph Lee, with other members including: TWH Hospital Governing Committee member, Mrs Viola Chan Man Yee-wai; Kowloon Central Cluster Clinical Director (Clinical Service) cum Chief of Service, Department of Medicine, Queen Elizabeth Hospital, Dr Patrick Li; Cluster Co-ordinator (Pharmacy), New Territories West Cluster, Ms Pauline Chu; Hong Kong West Cluster Director (Quality & Risk Management), Dr Seto Wing-hong.
The first panel meeting will be held on Thursday (April 23) and the Panel will finalise the report in about six to eight weeks for submission to Hong Kong West Cluster and Hospital Authority Head Office.
During the investigation period, the concerned staff would be transferred to perform other duties.
TWH would like to express the deepest condolences again to the family of the deceased patient and will provide the necessary assistance to the family. The other five patients are being treated and closely monitored in TWH with stable condition.
