Department of Family Medicine & Primary Health Care of Kowloon West Cluster announces a medication incident

The following is issued on behalf of the Hospital Authority:

     The spokesperson of Department of Family Medicine & Primary Health Care of Kowloon West Cluster made the following announcement today (April 17) regarding a medication incident occurred in the Lady Trench General Out-patient Clinic (LTGOPC):
 
     On April 17 morning, a pharmacist of the LTGOPC found that a tablet of anti-hypertensive drug (Diltiazem) in a pack of diabetic drug (Glibenclamide) during the checking process.
 
     Having noticed the incident, the Pharmacy Department of the LTGOPC immediately contacted totally 63 patients who may be affected in the incident to inform them to stop taking the Glibenclamide tablets and arranged for drug replacement.  Up till now, among the returned drugs, the anti-hypertensive drug is found in nine packs of diabetic drugs.
 
     With regard to this incident, our department had already expressed apology to all the concerned patients and would keep close contact with them.  The clinic will provide all necessary assistance to the affected patients.
 
     Our department has already followed the established procedures to report the incident to Hospital Authority (HA) Head Office and the Kowloon West Cluster management.  As the incident was related to human errors, the clinic management will follow the established HA Human Resource Policy and impose appropriate disciplinary actions.  Immediately after the incident, the Pharmacy Department has already reminded all staff members to be vigilance and strictly observe the protocols on preparation of medications.
